Every New Client will be required to read and sign this form upon Receiving grooming/bathing/sitting/boarding services, it will be kept on file for future reference:
Your animal is very important to us at Kay's Kare and would like to assure you that every effort will be made to make your animal’s experience as safe and pleasant as possible. Safety comes first for everyone, people as well as the animals, during the grooming/bathing/sitting/boarding process.
Current Vaccinations: Any new and existing clients being served in our salon must be up to date on all vaccinations. All dogs must be current on Rabies, Distemper, Parvovirus, and Bordetella vaccine. Please bring copies of vaccination records for our file.
Health or Medical Problems: Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. All medical expenses for veterinary care will be covered by the animal’s owner upon signing this contract/agreement.
Accidents: Although accidents are very rare, there is a risk when dealing with pets. Grooming equipment is sharp, and although we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quacking of nails, etc. In most cases, this can happen when a pet is wiggling or moving around. Your pet’s safety and comfort are our number one priorities. In the event an accident does occur, you will be notified of the accident. If the staff at Kay's Kare feels it is serious, and the owner is not on‐site, Kay's Kare will seek immediate veterinary care for your animal.
Owner Responsibility: The owner is solely responsible and liable for all acts of behavior of their pet. This may include but is not limited to, injury or death to pet, injury or death to another pet(s), or injury or death to a staff member or any other member of the public. Medical treatment expenses required by a staff member a member of the public or for another pet will be the sole responsibility of the pet owner.
Dangerous or Aggressive Animals‐ Refusal of Services: Kay's Kare has the right to refuse any services at any time. If your animal is too stressed or becomes dangerous to groom or bathe, Kay's Kare has the right to refuse grooming services, stop grooming services, or cancel grooming services anytime before, during, or after grooming. You will be charged a grooming fee for what was done up until that point.
Use of Muzzles: Muzzling does not harm your animal and protects both the animal and the groomer/bather. In some cases, muzzling may even calm a stressed animal, allowing the grooming process to continue. If an animal still acts in a way that is dangerous, Kay's Kare has the right to stop grooming services at any time. We do not muzzle unless your pet gives us a reason to. Other methods are used to calm your pet, muzzling is the last resort.
Medical Treatment Authorization: This release gives Kay's Kare full authorization to see medical treatment from the nearest veterinarian in the case of any medical emergencies while in the care of Kay's Kare. All veterinarian costs and expenses will be the responsibility of the animal’s owner.
Special Needs Handling Fee: An extra fee will be charged to your grooming/bathing charge if necessary. Some pets need extra time taken with them and will often need to have an assistant help with the process. Senior dogs may have difficulty with endurance while being groomed, and it may take a little longer than usual to go through the whole process; we may even have to take a break in between so that your dog is not in any discomfort, shy, aggressive, or extremely wiggly. Other special needs include aggressive and dogs with poor health conditions.
Matted Coat: Pets with matted coats need extra attention during their grooming session. Mats left in a pet’s coat only grow tighter and can strangle the pet's skin, and eventually tear it open. An evaluation of your pet’s coat by the grooming staff determines that a shave down is the best recourse for grooming your pet. This decision is made based on the stress and discomfort it would cause your pet if we were to attempt to brush or groom their coat. Mats can be very difficult to remove and may require the pet to be shaved. When necessary, removing a heavily matted coat includes risks of nicks, cuts, or abrasions due to warts, moles or skin folds trapped in the mats. Heavy matting can also trap moisture and urine near the pet’s skin allowing mold, fungus, or bacteria to grow, causing skin irritations that existed prior to the grooming process. Torn skin from mats can also harbor maggots. After-effects of mat removal procedures can include itchiness, skin redness, self‐inflicted irritations or abrasions, and failure of the hair to re‐grow. Shaved pets are also prone to sunburn and should be kept out of the sun until the hair grows sufficiently to protect the skin. In some cases, pets may also exhibit brief behavioral changes. Presentation is the best defense against matting by scheduling regular grooming appointments. There is an extra charge for de‐matting coats determined by your groomer.
Fleas/Ticks: If your pet has any fleas they will be given a flea bath at your expense of $5‐$15. If you do not want them to receive one, you may reschedule your appointment after the fleas have been resolved at home or veterinary care.
Grooming Enclosures: I agree and understand that my dog(s) may be placed in a grooming enclosure before, during, or after grooming.
Picking Up of Pet: If your pet is left at Kay's Kare longer than 2 hours past completion of their grooming, day boarding charges will apply.
Satisfaction: Your satisfaction is important to us. If you are unhappy for any reason and would like something adjusted, we will be happy to make any adjustments when you pick up your pet from his/her appointment. Once, however, you take your pet home from the appointment, any return visits will be treated as a new appointment and the client will be charged a grooming fee of some kind.
